Do I need a certificate OF DISABILITY from my orthopedic doctor to apply?

Most physical disabilities that impair activities of daily living are obvious (e.g. amputated leg, deformed and nonfunctional extremity) and do not need a certificate of disability from an orthopedic specialist. You can proceed to the Person with Disability Affairs Office (PDAO) in your city or municipality for your application for a PWD ID card.

When is it necessary to see my orthopedic doctor?

Only when the disability is not obvious will there be a need for a certificate of disability from your orthopedic surgeon

What if I have undergone treatment, can I still apply for a PWD ID Card?

If your disability has not been resolved despite the appropriate treatment, then you may request a certificate of disability from your orthopedic doctor. However, if the condition has been resolved, you cannot be considered as disabled.

Scoliosis is not automatically a disability. Please consult a certified Orthopedic Doctor for proper evaluation.

Kailangan ko bang humingi ng Certificate of disability sa aking Orthopedic Doctor para mag-apply?

Karamihan ng kapansanang nakaka-apekto sa pang-araw araw na gawain, ay hayagang nakikita (halimbawa: putol na binti o hita; mga kamay o paa na wala sa maaayos na porma at hindi nagagamit)  at hindi kailangan ng Certificate of Disability mula sa inyong Orthopedic Doctor. Mangyari po lamang na magtungo sa Person with Disability Affairs Office (PDAO) ng inyong siyudad o munisipyo para mag-apply ng PWD ID

Kailan ko kailangang magpunta sa aking Orthopedic Doctor?

Kapag ang kapansanan ay hindi hayagang nakikita, kailangang magsadya sa inyong Orthopedic Doctor upang makakuha ng Certificate of Disability.

Maaari pa ba akong mag-apply ng PWD ID Card kung ako’y sumailalim na sa Orthopedic Treatment?

Kung ang iyong kapansanan ay hindi nalutas, sa kabila ng angkop na Orthopedic Treatment, maaari kang manghingi ng Certificate of Disability mula sa iyong Orthopedic Doctor. Subalit, kung ang iyong kundisyon ay nalutas na, hindi ka na maituturing na may kapansanan pa.

Ang pagkakaroon ng Scoliosis ay hindi nangangahulugang ikaw ay may kapansanan. Mangyari po lamang na magpa-konsulta sa sertipikadong Orthopedic Doctor para sa wastong pagsusuri.
